At-home COVID test reimbursement information

At-home COVID tests purchased at an Express Scripts in-network pharmacy on or after January 15^th, can now be 100% reimbursed through Express Scripts. COVID tests are limited to eight tests per member per month. If one test box contains two tests, the limit is four boxes per member per month. A receipt and completed claim form are required to be sent to Express Scripts in order to be reimbursed.
