2019 Form 1095-C available in Workday

The 2019 Form 1095-C is now available in Workday for all benefits-eligible employees. Paper copies of Form 1095-C, will be mailed out in the next several days to those employees who did not select to receive their completed Form 1095-C in electronic copy only.

To view and/or print your 2019 Form 1095-C in Workday, select Benefits > View > My ACA Forms > View/Print.

Form 1095-C is required as part of the Affordable Care Act and it provides you with information about the health insurance coverage offered by Fairfield University and whether you enrolled in this coverage.
